PURPOSE OF THIS NOTICE
Title V of the Gramm-Leach-Bliley Act (GLBA) generally prohibits any financial institution, directly or through its affiliates, from sharing nonpublic personal information about you with a nonaffiliated third party unless the institution provides you with a notice of its privacy policies and practices, such as the type of information it collects about you and the categories of person or entities to whom it may be disclosed. In compliance with the GLBA, we are providing you with this document, which notifies you of the privacy policies and practices of Eaton & Berube Insurance Agency, Inc.
The GLBA further requires that we inform you that you have the right to prevent us from sharing nonpublic personal information about you with a nonaffiliated third party for any purpose that is not specifically authorized by law. Your right to prevent us from sharing nonpublic personal information about you with a nonaffiliated third party for a purpose that is not specifically authorized by law is called your right to "opt out". You may contact us for specific instructions on your right to opt out at any time.
In order to provide insurance products and services that respond to our customers' diverse needs, Eaton & Berube Insurance Agency, (E&B) collects certain personal information. We may, from time to time, have associates provide you with information on insurance products or financial services that may be of value to you. E&B is dedicated to protecting the confidentiality and security of nonpublic personal information that we collect about our customers. Duties and responsibilities of E&B employees include practices and procedures regarding the collection and handling of personal information about our customers to ensure adherence to the policies and practices outlined in the notice and compliance with Federal and State regulations under the Gramm-Leach-Biley Act (GLBA).

Collection of Personal Information
We get most of our information directly from you. The application you complete, as well as any additional information you provide, generally gives us most of the information we need in order to process your request for insurance coverages. Sometimes we may contact you by phone or mail to obtain additional information. Depending on the nature of your insurance transaction, we may need additional information about you or others potential insureds from outside sources such as motor vehicle records, loss information reports, credit reports, court records, medical information or other public records. For property insurance, we may send someone to inspect your property and a photo may be taken to verify information about its' value and condition.
